Catholic Charities Sponsoring Rural Wellness Training on February 18th
Sonora, CA...The Mother Lode Office of Catholic Charities is conducting a Patient Health Advocate volunteer training scheduled for Tuesday, February 18th. The goal of the Rural Wellness Alliance Project is to improve the effectiveness of interactions between Primary Care Providers and those over the age of 50 in an effort to increase adherence with medical treatment and improve health outcomes. The Project encourages older adults to self-advocate as well as improve their self-management skills...
Patient Health Advocate volunteers will provide clients with support, encouragement, access to community resources, and facilitate transportation involving medical needs, as well as promote positive self-advocacy. This project promises a gratifying volunteer experience and will certainly make a difference in the lives of those assisted.
The Rural Wellness Alliance Project is through the collaborative efforts of Sonora Regional Medical Center, Tuolumne County Public Health Department, and the Mother Lode Office of Catholic Charities. Support for this project is provided by a grant from the Robert Wood Johnson Foundation Community Health Leaders program.
